  • At Azimuth Healthcare, we want to take a moment to preview what’s ahead in 2025 while also reflecting on how we supported families and caregivers in 2024.? Looking ahead to 2025, we will be sharing a few upcoming announcements about new channel and distribution sales partnerships and an innovative app that will enable us to enhance our support for employers seeking to better support their teams. Here’s a preview of what our app will offer:? Our app will be designed to transform the employee experience by providing a comprehensive digital health hub for employees and their families. More to come on the official launch but here are some key features of the app. The app will allow family members to be connected (regardless of whether or not that family member is included in the employer-sponsored benefit) enabling seamless and meaningful coordination and navigation of health and wellness services. Acting as a one-stop shop for wellness and health, the app aligns with employers' cost-containment goals by assisting in preventive care, avoiding unnecessary expenses, and reducing hospital readmissions. It connects employees to their benefits with ease, offering priority access to critical resources while improving overall health and productivity. Beyond supporting physical health, the app will foster corporate culture by alleviating the stress of family health planning, empowering employees to focus on their work and well-being. Reflecting on 2024, we are proud of the strides we made in supporting family caregivers through innovative solutions, impactful partnerships, and insightful resources shared on our Care Hub. We want to give a heartfelt thank you to our team for providing exceptional support and guidance to families, especially?Ingrid Kaiser?and?Joe?and?Pattie Savarie.     November is Long Term Care Awareness Month, which is a great time for me to share that?I am now an advisor to Azimuth Healthcare, led by Robert Witham (He/Him) (Military Veteran) and Darren Moorehead, MSHA, FACHE. As I previously posted, they are leading the charge to help people get the care they need for themselves or their loved one's, by navigating, advocating, and coordinating everything?involved, across multiple use cases.     ?? Update on Azimuth Healthcare! In a post earlier this year, I announced the official launch of Azimuth Healthcare. And I now want to update everyone on how we're currently supporting families, their providers and ultimately strengthening communities. ?? ??Some have said that we're the "connective tissue" for families and the communities we serve. Hearing these comments is what keeps us going! ??Our team of expert coordinators, navigators and advocates are driven by passion to help others in their communities and our simple mission: ...Love your loved one, and we take care of the rest... ??It's important to recognize some of our AMAZING partners that have worked with us and are absolutely part of the "connective tissue". ?? ??To date, here's just a few examples of how we've helped! - We've met families where they feel most comfortable...and that's in their homes, sitting across the dining room table, discussing how to better help their families navigate our fragmented healthcare system. ?? - We've helped reduce almost $50,000 in unnecessary healthcare costs by helping reduce avoidable ER Visits, Readmissions, etc. ?? - We've coordinated in-home personal care with Amada Senior Care of Richmond. Thanks for the support Jay L. Mann ?? - We've coordinated in-home urgent care with DispatchHealth ?? - We've coordinated in-home care with Enhabit Home Health & Hospice ?? - We've explored care and support options with Suncrest Hospice (Thank you Mary Catharine Ginn Kolbert), Marquis Health Consulting Services (Thank you Blair Lawson) and Amramp ?? - We've explored support and planning options with one of the most important services for families with Reamey Belski! ?? - Navigated Veterans, Medicare and Medicaid benefits ???? - Advocated for better clinical care in post-acute facilities ?? ??And here are the services we provide to all of our clients: - 24/7 family support and resources - Researching and scheduling physician appointments - Attend physician appointments in-person or virtually as needed? - Organizing transportation assistance as required - Maximizing and coordinating insurance benefits - Coordinating social isolation prevention activities - Coordinating errands, including groceries, prescription pick-up, etc. - Evaluating need for mobility aids, including ramps and stair lifts - When appropriate, set up skilled home health and non-skilled personal care aides ??Thank you to our AMAZING and DEDICATED team members, this would not be possible without you!??

  • What to do when it seems hopeless... Mental health illnesses can sometimes lead to suicides. Here are some facts and some ways to be prepared to help someone in need. According to the CDC, suicide is preventable. It is a serious public health problem that can have long-lasting effects on individuals, families and communities. Preventing suicide requires strategies at all levels of society. This includes prevention and protective strategies, learning the warning signs and committing to social change. By strengthening household financial security, housing stability, reducing substance abuse and limiting access to lethal means we can begin to impact the risk.? Improving access and delivery of suicide care - covering mental health conditions in health insurance, increasing provider availability and providing rapid access to help - we can further reduce the risk. Implementing social-emotional learning programs, supporting parenting skills and education programs as well as identifying persons at risk and providing therapeutic approaches can still further reduce risk. Be aware of potential warning signs, such as: - Withdrawing from friends - Saying goodbye - Giving away important items - Taking dangerous risks such as extremely reckless driving - Displaying extreme mood swings - Eating or sleeping more or less. How can you help someone who may be suicidal? - Listen to the person - Accept what they are saying and take them seriously - If they start talking - do not interrupt or add your feelings to the conversation - Let them know you care and are concerned.? - Do not judge. - Say something simple such as: “Please don’t hurt yourself. I don’t want you to kill yourself, I would miss you terribly. “ - Saying something like ”my life would be less full without you” can help the distressed person think about their reasons for living.? -Connect them to resources, such as the suicide hotline, their doctor or a mental health professional.? - If you think they are in extreme danger, reach out to the hotline yourself. - Stay with the person and try to connect them with a trained crisis counselor via the hotline.   • ????? Happy Pride - Advance Care Planning for LGBTQ+ Community ????? It’s estimated that there are over 2.7 million LGBT adults over the age of 50 and 1.1 million 65 and older.?The numbers are projected to reach over 5 million in the next few years. The importance of Advance Planning for LGBTQ+ older adults cannot be overstated. There are a variety of reasons why this is especially important to the LGBTQ+ community and they include: - Laws and a legal landscape that may not be supportive of the LGBTQ+ community in advance care and some laws maybe harmful to end of life wishes of an LGBTQ+ older adult - Family may not be the preferred decision maker - May not have traditional caregivers in place It is not just important to tell people in your personal circle what your wishes are. These wishes can range from Health Care Proxy and Living Wills; to what do you want after death and how do you want your life to be honored. These are very personal issues to everyone but it is especially important for LGBTQ+ older adults to have legal documents specifically expressing those desires.??Most older LGBTQ+ older adults can all share with you a story where of individuals who have a had distant family come in and take over for the person who is dying not honoring that individual??or of having express wants and needs discarded by health care professions.? Things for LGBTQ+ older adults to consider in Advance Planning include but not limited to: - Health Care Proxy- each state has different laws for handling this if no health care proxy is specified - Living Will - Financial and Estate planning - Emergency Folder? - Engaging Death Doula - End of Life planning? - After death As we continue to age in an ever-changing landscape for the LGBTQ+ community there are resources that can help ensure we can age with dignity and our values, beliefs and wants be honored and celebrated.
