Issues Correction for lnfusomat Space Infusion System/ Large Volume Pump, Infusomat Space Large Volume Pump, Wireless, and lnfusomat Space Large Volume Pump, Non-Wireless BATTERY PACK Due to Faulty Occlusion Alarm B. Braun Medical Inc. (US) is expanding?an earlier recall?to add lnfusomat Space Infusion System/Large Volume Pump to the already included models,?due to the risk that an alarm may sound to indicate a blockage (occlusion) when no occlusion exists. This will cause the pump to stop delivering medications, including critical high-risk medications like those that constrict blood vessels and raise blood pressure (vasopressors). The FDA has identified this as a Class I recall, the most serious type of recall. This recall involves correcting devices and does not involve removing them from where they are used or sold. Diagnostic Excellence is a critical patient safety issue. Nearly?800,000 Americans die?or are permanently disabled by diagnostic error each year, making it the single largest source of serious harm from medical mistakes. In observance of World Patient Safety Day, the Centers for Disease Control and Prevention, Agency for Healthcare Research and Quality, and Centers for Medicare & Medicaid Services have released new resources for hospitals to improve the accuracy and timeliness of a patient’s diagnosis to provide better treatment and save lives. The?Core Elements of Hospital Diagnostic Excellence Programs?describe the key framework and actions that can be implemented to improve diagnoses for hospitalized patients. These include ensuring that diagnostic tests are ordered, interpreted, communicated, and acted upon appropriately, bringing the nation closer to zero preventable harm. This framework was informed by expert knowledge from the clinical and patient community. The Hospital Diagnostic Excellence Assessment Tools provide?priority examples and?additional examples?to help hospitals implement the Core Elements. Improved diagnoses will drive progress in other patient safety priorities, such as antibiotic use and advances in the timely and accurate diagnosis of sepsis. The?Medical Test Checklist for Hospitalized Patients and their Caregivers?empowers patients and their loved ones to ask important questions during a hospital stay. Patients, families, and caregivers are vital in the effort to achieve greater and sustained diagnostic safety. Correct and timely diagnosis can save lives.
