Are you a compassionate and dedicated Registered Nurse with experience in hospice care? We are seeking a skilled Hospice RN Case Manager to join our team and provide high-quality, patient-centered care to individuals on their final journey in life. In this role, you will be responsible for coordinating and managing care plans for patients and their families, ensuring comfort, dignity, and support during this critical time. Responsibilities - Develop, review, and implement healthcare plans for patients. - Maintain knowledge of State/Federal regulations - Assisting with on-call nursing duties - Perform comprehensive nursing evaluations Qualifications - Bachelors degree in Nursing (other related degrees may be accepted) - At least one year experience in one of the fields: Hospice, Palliative Care, or Home Health. - Current RN License with the state of Texas. - Ability to work with dying patients and their families.
