Red Flags in Fall Protection Modifications: ?? Altering Anchor Points: Never modify or create makeshift anchor points. These must meet strict strength requirements (e.g., 5,000 lbs). Using pipes, railings, or other unapproved structures is extremely hazardous. ?? Modifying Harnesses or Lanyards: Do not alter or repair fall protection equipment. Any damage or modification renders it unsafe. Only manufacturer-approved repairs or replacements are acceptable. ?? Circumventing Guardrails or Barriers: If you're creating a "shortcut" that bypasses established guardrails or barriers, you're directly increasing fall risk. ?? Improvised Platforms or Scaffolding: Building makeshift platforms or scaffolding without proper engineering and safety checks is a major violation.
