????We are looking for a Nurse Practitioner to join our team! This role is both home based, and facility based geriatric primary care, with emphasis on advance care planning, palliative management of symptoms, and aging in place. Duties will also includes integration with the hospice team to maintain continuity and performing visits for hospice patients to help meet face to face requirements, and symptom management visits when appropriate.
