The U.S. Chemical Safety and Hazard Investigation Board (CSB) released a?new safety video?on its investigation into the fatal release of hydrogen chloride at the Wacker Polysilicon North America facility in Charleston, Tennessee, that occurred on November 13, 2020. Over the summer, the U.S. Chemical Safety and Hazard Investigation Board released an update for their ongoing investigation into the 2023 explosion and toxic chemical release at the Honeywell facility in Geismer, LA. On January 23, 2023, a reboiler at the facility catastrophically exploded, leading to a release of over 870 pounds of HF and nearly 1,700 pounds of chlorine gas. Nearby highways were closed, workers had to shelter in place, and the resulting property damage was estimated at $4 million. In this CSB update, it was revealed that an MI inspection in September 2020 estimated only 1.1 years of remaining service life for the reboiler shell. After a visual inspection in October 2021, it was recommended that the shell be replaced at the next shutdown, and in January 2022 a project began to pursue a replacement, but ultimately the replacement had not been purchased or installed by the time the incident occurred on January 23, 2023. The Center for Chemical Process Safety (CCPS) has just released a new monograph titled "Key Principles for Operating Procedures". The seven key principles CCPS defines are: 1. Develop Operating Procedures required to run and maintain the process 2. Ensure Operating Procedures cover all modes of operation 3. Write Operating Procedures that are concise, accurate, and user friendly 4. Keep Operating Procedures accurate, current, and accessible 5. Train operators on proper use of the Operating Procedures 6. Ensure adherence to Operating Procedures 7. Manage short-term changes to Operating Procedures Each key principle is accompanied by an incident that highlights the principle's importance, and actionable steps for both leadership and implementers/users.
