    ?? NMES 101: The "All-or-None Law" Ever wonder what's really happening when we use electrical stimulation? Let's break down this fundamental concept that's crucial for effective dysphagia treatment. Here's the simple truth: A motor nerve is either activated or it isn't - there's no in-between. Think of it like flipping a light switch. When the stimulus intensity is sufficient, the nerve fires at its maximum response. If it's not sufficient, there's no response at all. That's why it's called the "All-or-None Law." This is where terms like "submaximal" and "maximal" often get muddied in our field. Let's clear it up: Electrical stimulation is either sensory or sensorimotor. Sensory is submaximal - meaning there's not enough intensity for the nerve and muscle to achieve that maximum response. But when you turn the intensity up high enough to depolarize that nerve, that is when you get a true motor contraction. This is exactly why Ampcare ESP is designed to reach the necessary intensity (100 mA) while maintaining patient comfort.     Who needs a good story of triumph that will have you throwing a fist into the air in celebration for a fellow SLP? We've got one to get you jazzed about dysphagia therapy today! Kimberly B. works in a hospital and received a consult for a woman in her late 30s with a brainstem stroke... She completed a modified barium swallow study, which ended after puree trials. The patient was unable to clear most of the bolus despite her best efforts, leaving her with puree residue in her valleculae and pyriform sinuses. Using the findings from her MBS, Kimberly crafted a dysphagia treatment plan using Ampcare ESP with lingual and pharyngeal exercises 2-3x/week. Two weeks later, the patient was scheduled to be discharged, so Kimberly completed a repeat MBS. The result? SIGNIFICANT improvements!     How many times have you done an MBSS or FEES and found impaired laryngeal vestibule closure resulting in aspiration? Sure, there may be multiple factors at play that result in reduced bolus efficiency or airway protection. If we could get the airway closed efficiently and timely, how would that change the overall safety of the swallow? That's exactly what Ampcare does! The neuromuscular electrical stimulation pulls the airway superiorly and anteriorly. The patient is prompted to swallow against this resistance, creating an effortful swallow and improving laryngeal vestibule closure.     Not to burst your bubble but... If you don't SEE motor contraction, you're not getting a motor contraction... You may have heard Rick or Russ say that at ASHA or in the course or on one of the IG Lives, but what the heck does it mean? What's the difference between sensory stimulation and motor stimulation anyway? ?Sensory stimulation is simply that... sensory. You feel the tingle. It sends a stronger signal to your brain via the afferent pathway. It's all good and fine. ?? Motor stimulation takes it one step further and creates a muscle contraction. When you think motor, think muscle movement. You will visibly ??SEE?? your patient's neck or face MOVE. They will feel their muscles contract and as a result their neck or facial structures move. Motor stimulation is THE goal of neuromuscular electrical stimulation (NMES). If you think you're getting motor stimulation and you're not seeing movement before your very eyes... you're NOT utilizing true NMES. You're utilizing sensory stimulation, which as we said is all good and fine. BUT, which do you think is more effective for patients with dysphagia who have impaired movement of their hyolaryngeal structures?
