    Post for ID/HIV enthusiasts: My take homes from CROI 2025 were mainly about PrEP- it was exciting! LA CAB PrEP performed very well in a large real world (Kaiser Permanente) US cohort ( n=141) and in the PILLAR study (n=201). There were no incident infections, persistence was very high, >90% coverage and it reached people who had never taken PrEP (25% in KP cohort). In a diverse Brazilian PrEP cohort all of whom were tested for HIV (genXpert test at baseline), 86% chose CAB LA. Adherence on CAB was very high, coverage was higher than oral PrEP and there were no incident infections on CAB. 6% discontinued CAB mainly due to side effects. VL monitoring in 084 study had low PPV as it did in 083. Achieving viral suppression in people with INSTI resistance was possible using INSTI-based regimens either directly or after period of suppression on NNRTI/PI regimen. LEN: In adolescents in PURPOSE 1 , there were 0 infections and PK was similar to adults. The first YEARLY LEN (an IM not SC) injection has been studied in phase 1 in 20 people.     Truly phenomenal talk in the #croi2025 opening session from Rebecca Denison Diagnosed with HIV in 1990, Rebecca shared her incredible journey, joining ACTUP, founding WORLD (https://lnkd.in/ep-ZT62b), and having HIV-negative twin daughters (one of whom was in the audience, alongside her dad & the doctor who delivered her & her sister!!). Rebecca called out the misinformation peddled by the new US Secretary of Health including his assertion that the "gay lifestyle" caused AIDS, not HIV....."Hey RFK, I never used poppers" She spoke eloquently of her own privilege, her freedom from institutional censorship & the irreversible harm of the ongoing decimation of Global HIV funding.     Post from San Francisco: I have been re-posting recently to amplify the voices of my US colleagues about the war on science and public health that is taking place in the US and the cuts to public funding for PEPFAR HIV programmes supporting treatment and HIV prevention for millions worldwide. It seems that some UK colleagues are not aware of the extent of other domestic policies. So read on for more… Unlike federally employed US scientists I have been able to travel here(they are subject to a travel ban). I am able to present data that reports on gender identity (this is now banned for government employees as part of the war on ‘woke’ which prevents all research about transgender people and stops them receiving gender affirming hormones). National datasets with data or guidelines on gender identity and sexuality have been removed off CDC (centre for disease control ) government websites in recent months. I am here to listen to science on vaccines at a time when high profile meetings on flu strains have been cancelled/ postponed here. So I am at CROI feeling very fortunate to be able to support science by presenting late-breaking data on a novel HIV drug (Islatravir) with potential as a core agent in future long-acting therapies.     There has been lots of news these past weeks about US foreign assistance funding and our efforts to reverse the freeze. As many know, AVAC and Global Health Council, along with a number of partners, are part of two lawsuits that have been working their way through the courts to protest the freeze on foreign assistance. A timeline with links to lots of resources is available here:?https://lnkd.in/gSYZzngd. On Wednesday, the US Supreme Court ruled that the government did need to pay their bills for all of US foreign assistance – but they did ask District Court Judge Amir Ali who is overseeing the cases to provide more clarity about a feasible plan and timeline to make the payments. Then Thursday, Judge Ali held a four-hour hearing to review not just the payment timelines but also the overall case about pauses and terminations. At the end of the hearing, he ordered the government by Monday, March 10, to pay all payment requests and immediately release line of credit draw-downs from plaintiffs in both the AVAC and GHC cases. Monday is also the last day of the Temporary Restraining Order, and we expect Judge Ali to make a further ruling on payments to all organizations owed money, and not just the plaintiffs. To be clear, though, AVAC, GHC and partners did not bring these suits just to get their money; these suits are about calling the freeze illegal and restarting foreign assistance. So, while Thursday's ruling is definitely a positive step, it is just a step – and there are many questions about what happens next. And while we expect initial payments to begin flowing between now and Monday, it is not all clear what else will happen, whether more terminations will be reversed, and if the government will comply – which is the larger constitutional crisis.     Voluntary medical male circumcision (VMMC) is no joke, and it is no scam -- in fact, it's "one of the best [HIV] prevention investments we have." For almost 20 years, VMMC has been a mainstay of the HIV response, thanks to PEPFAR's leadership. Tragically, the new president ridiculed it in his address to Congress and way too many members laughed about it. I wish this new administration and the Republican Party would be like PEPFAR -- data-driven, evidence-based and commitment to making the US and the world safer, stronger and more prosperous. https://lnkd.in/eGndTki6
