Is it time for HLA-DQ matching in lung transplantation? We begin 2025 with an important finding: HLA-DQ plays a crucial role in lung transplant outcomes! In a cohort study of 128 lung transplant recipients, we found that both HLA-EMMA and PIRCHE-II DQB1 mismatch scores were strongly linked to: ? Faster development of anti-HLA-DQ antibodies ? Worse graft survival Next, in a nested case-control study of 69 CLAD-RAS patients and 69 matched non-CLAD controls, we showed that: ? Anti-HLA antibodies, particularly those targeting HLA-DQ, and ? Antibody-mediated rejection were major risk factors for developing RAS. These findings deepen our insights in why patients may develop chronic rejection, and suggest that HLA-DQ matching during lung allocation could help lower CLAD-RAS prevalence and improve long-term graft survival. ??We are happy to announce that the article with the results of the study “Donor-specific anti-HLA antibodies (DSAs) in patients undergoing allogeneic hematopoietic stem cell transplantation from mismatched donors” has been published on Blood Transfusion Journal. ??GITMO and AIBT conducted this study among Italian Transplant Programs, with the aim of analyzing the policy foranti-HLA antibody and DSA detection and approaches for positive results in terms of donor selection criteria and desensitization strategies in hematologic patients who were candidates for HSCT. A clear scenario of DSA management policies adopted by the Italian Transplant Programs will allow for the definition of shared consensus strategies.
