Preventing Newborn Misidentification: A Critical Issue Did you know that 11% of newborn medical errors involve misidentification? Without standardized naming conventions, hospitals risk placing orders on the wrong chart or mis-linking medical records. AHIMA recommendations on reducing errors were included in a recent AAP News article. Adoption of standardization has been slow due to outdated systems and workflow barriers, but patient safety must come first! ??
