AHI Program Spotlight: LAST up in our series of posts highlighting the valuable programs offered by AHI is our Collaborative Model of Community Medicine program! This pilot program is a partnership between AHI, Glens Falls Hospital, HCR Home Care, and Cambridge Valley Rescue Squad, Inc., supported by the Mother Cabrini Health Foundation. This program has launched in Washington County and brings essential healthcare services directly to patients homes. AHI in the Community! Our health insurance enrollment assistance (EASE) and Health Home care management team members were present this past weekend at the Ballston Spa Central School District Wellness Expo! The expo consisted of a variety of agencies that provide services around healthy living, mental health, fitness, healthy eating, and more. You can usually find AHI tabling at events like this all over the region!
