How to proactively prevent hospitalizations for older adults. This is a true story. Read on it gets interesting… I’m not using real names, and my son is really into a Spider-Man atm, so I’m pulling names from that. MJ is a superstar caregiver. (Btw I’m not a fan of the title “caregiver” we do so much more, Comprehensive Home Support Specialists is closer to what we do) MJ is helping Peter recover in the comfort of his home. Peter has trouble communicating with MJ and is becoming forgetful. MJ helps Peter with a lot, cooking, cleaning, bathing, re-organizing the home to make daily activities easier and safer, errands, groceries… and more… you get the picture But MJ is also doing something, some may find boring… but it’s extremely important. Collecting health care data. This data lets MJ’s care team back at base know if something is off… we call this change in condition protocol. Today, something is off… lethargy, loss of appetite, drinking less water. MJ reports the findings back to base. We contact Peter’s Primary Physician and get an appointment. Peter’s Primary finds a Urinary Track Infection. Peter recieves treatment. Why is this a BIG deal… A 2024 Journal Of Infection study. Reports UTIs frequently cause hospitalization and dealth in people living with dementia, the highest risk in people living with dementia and diabetes (like Peter) delayed treatment is further associated with high mortality - Infectious Disease Practice: Volume 88, Issue 6, June 2024 Here’s how we do it… Daily assessments and reporting Medication management Fall Prevention Nutrition and Hydration Chronic Condition Management Robust Caregiver support and Client Education Social and Emotional Support Strong Communication with Health Professionals These strategies reduce hospital admission, enhance quality of life, and strengthen trust with clients and their families.
