1 Define the problem

The first step to improve problem identification is to clearly define the problem you are trying to solve. This means stating the problem in a specific, measurable, and realistic way. For example, instead of saying "the machine is not working", you could say "the machine is producing defective parts at a rate of 10% above the acceptable limit". By defining the problem, you can narrow down the scope, set the criteria, and communicate it to others.

2 Gather data

The next step is to gather relevant and reliable data about the problem. Data can help you understand the current situation, identify the root causes, and evaluate the potential solutions. You can use various sources and methods to collect data, such as observations, measurements, records, interviews, surveys, or experiments. However, you should also be careful not to gather too much or too little data, or to rely on assumptions or biases.

After ensuring the scope and definition of the problem the data gathering is the most important part as ways of analysis may be adjusted later as long as the right data to sufficiently describe the problem events are captured. The article states to “identify the root cause” and that’s great if known, but if it’s not - then performing a FMECA (both process, equipment and controls) should be performed with all SME’s and stakeholders. That will ensure that the possible inputs, system requirements and behaviors (at least intended) are known and agreed ahead of time. This also may help having the same team generate ideas on data collection approaches and what information will be useful to help the other groups implement any discovered changes.

3 Analyze data

After gathering data, you need to analyze it to find patterns, trends, correlations, or anomalies. You can use different tools and techniques to analyze data, such as charts, graphs, tables, statistics, or software. The goal is to interpret the data and draw meaningful conclusions that can help you solve the problem. You should also verify the accuracy and validity of your data analysis and check for errors or inconsistencies.

Es clave el uso de la estadística y variables a partir de una base de datos fehaciente que te permita de forma “libre” e integra tomar decisiones y dar el peso específico a cada causa, cada variable. Si los datos no son verídicos e imparciales la toma de decisiones desde esta etapa nos lleva a otra falla .

You should also consider the feasibility, effectiveness, and impact of each solution, as well as the resources, constraints, and risks involved. You can also involve other stakeholders or experts to get feedback or suggestions.

The final step is to implement the chosen solution and monitor the results. You should plan and execute the solution in a systematic and timely manner, following the established procedures and standards. You should also document the process and report the outcomes. Understanding the problem is the first step in identifying problem and driving a solution to your problem solving skills and unfortunately many problems remain unsolved because there has not been a focused effort to really understand and identify the problem itself, however good problem solving requires you to firstly understand the real problem you need to solve before you attempt to come up with any solutions. Often, the minute you identify the correct intent of the problem, a solution more easily follows. The more problems you solve, the better you get at it. You may get some wrong, but that's okay. Use the five steps, taking your time on each one to evaluate your decisions and ask for support from other team members when it's needed.

Considero que en cada caso, cual fuese, es importante hacer el planteamiento de invertir el tiempo, esfuerzo y todo el método descrito en este apartado en un prototipo de causa raíz, que nos permita resolver las condiciones, circunstancias, variables, elementos físicos, decisiones humanas y de sistema de cada caso. Cuando tenemos la capacidad de resolver la causa, el caso ha tenido más probabilidades de éxito que de una recurrencia, aún más cuando el coste de resolverlo reduce los riesgos e impactos a la integridad de las personas, instalaciones y el medio ambiente.
