This is one of the common problems we face throughout the world. Everyone has tried what you have listed including standardised protocols and effective communication. Despite all these mistakes do happen. The best way to prevent any errors and optimise flow is for a senior nurse in each shift to be incharge of all patients and have minimal clinical duty for her - to oversee these things and that person can be simultaneously evaluated for her leadership skills too. That way efficiency improves. Of course use of technology with AI to pick up early signs which can me missed is invaluable

Understand the capacity of the overall system and where the bottlenecks (constraints) are. Bottlenecks not only impact the whole system, but the stress at them can increase the likelihood of errors occurring. Start with the constraint that impacts the overall system the most and work your way back. Streamline processes and eliminate waste before adding more resources. Next, understand the variation in the system over time. What variation can you control? Level load where you can and then ensure that you have enough flex in the system to be able to cope with periods of high demand and a business intelligence system that helps you understand where those peaks are. A system running at the right capacity is a safer system.
