En el análisis de fallas y riesgos modernos, la más alta prioridad la tienen los modos de falla que pueden tener un impacto en la integridad física de los operadores de nuestra planta, los operadores en la planta de nuestros clientes y en los usuarios finales de nuestro producto. Uno de los aspectos más importantes de la reducción de riesgos en fallas de seguridad es la implementación de controles de prevención que puedan eliminar la causa de la falla antes de que esta ocurra. Este enfoque debe ser contrario al clásico intento de detectar la falla una vez que ésta se ha presentado. Un elemento clave en las acciones de prevención es la determinación de causas controlables.

En distintos grupos he notado que la mayoría de los incidentes de seguridad están relacionados más con actos inseguros que con las condiciones. Y, siendo los trabajadores la última capa de protección del esfuerzo de las organizaciones, es menester despertar en todos el deseo de cuidarnos. La seguridad no debe ejercerse por mandato sino por convicción de cada individuo; y en este punto el reconocimiento a los motivadores de cada individuo es el único camino para llegar a sus emociones.
