We often face this problem at 3D Vector Inc. and whenever possible we try to discuss any design changes with our clients in-person or via a live video call, while documenting any changes with a detailed summary email after the meeting. Face-to-face interaction with the client often helps eliminate ambiguity and rework. It's sometimes easier and faster to use electronic means of communication, but our principal rule is to revert to human interaction whenever discussing any changes to ensure that we are on the same page with our clients and their expectations.
