In practical for quality assurance team there is always opportunities wherein miscommunication in quality assurance at every point of ensuring quality of the product and services.sometimes design can misleads by providing wrong drawings or data.or supplier quality gives wrong information about changes implemented or even duplicate supplier product mix or wrong information about production change or BOM mixed,these issues gives wrong direction for problem solving,in such cases instead of blame game let's understand where process goes wrong instead of people if problem solving leads to training inspection or additional check there miscommunication or misleads happened,this simple technique resolved all miscommunication.
