Evolving scope is an issue in itself. If it is constantly evolving I would try to find the root cause of that. Is it a result of ineffective quality specifications and acceptance criteria? Or is it that some influential stakeholder/group is pitching in some Wishlist? As long as the Wishlist is not formally approved by the project board it won’t impact the scope. As for the risk monitoring, I would analyse the risk register for any existing risk that could contribute to scope creep and understand if the effective treatment is planned and is in effect. Maintaining constant risk monitoring should be carried out through out the life of project and surly as agreed in the risk management approach.
