You're faced with a mass casualty incident. How do you decide which medical supplies to prioritize?

During these crisis events, teamwork is essential in order to defuse the crisis at hand and implement immediate solutions for containment. Prevention, Training and efficiency are key factors in such protocol undertakings.

One thing I’ve found helpful during my time working as casualty officer at the emergency department was triaging my patients with the help of the medical team. When your triage well it gives you insight of what’s needed to cater for your patients, that will enable you assign the resources you have and demand for the resources you need.

Evaluate Available Resources 1. Inventory Supplies Quickly assess the quantity and type of medical supplies available. Determine if additional resources are accessible or if resupply is possible. 2. Allocate Based on Triage Match the most critical supplies to the needs of the most urgent patients, ensuring that limited resources are used where they can have the most impact.

Instead of listing every supply individually, I’ll group them by category. Triage and Patient Assessment includes trauma tags, writing utensils, and PCRs. Airway Management covers oxygen and adjuncts. Circulation and Hemorrhage Control involves tourniquets and IV kits. Other categories include Wound Care, Immobilization Devices, Medications, PPE, Decontamination supplies, Stretchers, Power and Lighting, Food and Water, Waste Management, Protocol Books, and Personnel. The ability to resupply quickly is crucial. The above supplies should be ready at a moment’s notice, though not every agency can afford fully stocked MCI trailers and the people to maintain them, so they stay updated and useable.

There is a reason on why we, in the field of Emergency Management, have a preformatted framework for responding to disasters. Whether this is a natural or human caused incident the FEMA guidelines are in place for agencies to follow. The question comes down to what part of NIMS / ICS are you involved in? As a Police Sergeant I have initiated and maintained the ICS in accordance with NIMS for numerous critical incidents. Not being a medical professional I have contents related to simple triage such as but not limited to torniquet, trauma bandages, CPR mask, and medicinal alcohol in my gear bag. The intent is to conduct First Aid while transporting injured to medical professionals.

In an MCI, communication breakdown in a logistical manner can be crucial in the response effort. For example, if Fire/EMS personnel communicate to the incident commander or delegate that more tourniquets or TCCC-ready kits are needed because they are being depleted at the scene, this information is critical to sustaining casualty care and/or saving a life. Further, if this information is communicated to the proper point of contact or the adequate channels to replenish the stock of supplies well the obvious outcome is apparent. Cross-communication (up/down and laterally) is critical for the ICS/NIMS framework and could drastically affect the recovery phase for all parties involved.

The only way to be as prepared as possible for an unknown MCI is through preparation. Interagency MCI training exercises reveal our shortcomings, mistakes, and lapses in judgements. However, the more we train, the more prepared we become for events that may or may not happen during our careers. Embracing mistakes and learning from them are crucial to improvement. I’ve participated in over 50 MCI trainings across military, FEMA EMO ICE, and college settings. In one GOAA MCI training, an Orlando FD medic had the green triaged, sit down right in front of him once they reached him, preventing them from wandering and ensuring proper reassessment—a simple yet brilliant decision to maintain order and to be methodical in a place of pandemonium.
