In my experience it's normally other small factors that are the route cause of the team not performing. These as are all good points but a route causes analysis should be carried out alongside this.

Without one, plans can feel confusing and hard to follow. By breaking down tasks, setting deadlines, and figuring out who's responsible for what, the action plan makes sure everyone knows what to do. This makes teamwork easier and helps get things done faster. Tactics focus us on the most important tasks first. Helping our team use their time and energy wisely. Action plans set an expectation to check in on the plan regularly and to make changes as needed. Keeping things on track, even when things don't go as expected.

As a manager you should enable the feedback process but be willing to step in and direct it back on track if it starts going down the blaming route. If not controlled it can do the opposite to what you are trying to correct. It's a hard balance to let people speak and feel listened to without degrading other people if they believe they are at fault.

The final step in operational planning is to evaluate the results of your team's performance. This means measuring and comparing the actual outcomes with the expected outcomes, based on the indicators of success that you defined in the action plan. You should share the results with your team and celebrate the achievements, as well as identify the areas for improvement and the lessons learned.
