Struggling is often a manifestation of not being able to achieve the goals set out in the first place. Adding more goals simply adds frustration and sets up for failure. Instead have a conversation based on 3 simple questions: 1. What's missing? What's missing for the person that, if it weren't missing, they'd be performing closer to their potential. 2. Where do we go from here? Given the current situation, what is the intention for the next few steps? Are the original intentions still relevant? If not, what is a better intention? 3. What can I (the manager) do to support you (the person) and your success (the task)?

This is probably the single-most difficult thing any manager has to do. Learn to do it well and it will pay dividends forever. Personally, I have always appreciated when I receive both constructive and destructive feedback--can learn a lot from both. In my opinion, the key is to have formed some relationship of trust and respect with the individual beforehand, leave out the emotion, deliver as neutrally as possible, couch positively--with concrete points and a plan to improve and grow (with check points). In this particular context, if might be a good idea to explore if the report is fit for the role going forward. The best feedback could possibly be for the individual to move on to something more appropriate.
