I can share my past experience for the same. one of the parameter from the soil report was out of the limit which was submitted to the consultant without confirming the test results value. The following correction and corrective actions were to taken to avoid recurrence. Correction: A spreadsheet was prepared with all requirements and set values ( less or more ) for formula against each test values. so that when you enter the test values we will come to know that the test is passed or failed. Corrective Action : Instruction had given to the Independent Lab to take care of all test parameters as per SOP / Standard procedures. Informed to execution team to mix and prepare the soil well to ensure it’s homogeneous before tacking sample.

The standard operating procedures for organizations are slow to change unless there is an extreme outlier that goes against the SOP. Need to perform root cause analysis. Post the root cause analysis need to devise appropriate CAPAs followed by updation and automation of procedures if required followed by training. Meanwhile interim action should be in place to ensure same incidence does not recur which may involve incorporation of checklist based on review of docs and system.

Frequent sharing of test findings with the team and root cause analysis of any variances will result in early patterns to be detected and hence scope for improvement.
