Your plant processes are facing a sudden increase in defects. How will you effectively tackle this challenge?

Step 1: Find out the root of the problem by doing Root cause analysis(RCA). Step 2:Check the Monthly Preventive Maintenance (PM)Check list if it is done or not. Note:-Try to keep the PM percentage minimum 85%. Step 3: After you find the Root Cause do strategic planing with the experts on how you can reduce this technical failure's or defects. Step 4: After your planing is done train your staff on how to execute your plan efficiently and effectively. Step 5: After the plan is implemented start having Monthly Review Meeting (MRM) to have clear picture on the current status of the defects. This how you can effectively tackle this challenge.

Not keen on the term “root cause analysis” as I find it generates the single cause mindset. In my experience this is rarely, if ever a single route cause. Root causes analysis is the minimum I would expect to see.

When defects spike, I first halt production to prevent further issues, isolate defective products, and notify key stakeholders. Next, I assemble a cross-functional team from production, quality control, maintenance, and engineering to gather diverse insights. We use tools like Fishbone Diagrams, 5 Whys, and Pareto Analysis to identify root causes, inspect equipment, and review recent changes. Immediate fixes might include recalibrating equipment or adjusting processes, while long-term solutions involve revising SOPs and enhancing training. Monitoring effectiveness and implementing preventive measures helps ensure we prevent future defects and strengthen our system.

RCA is indeed first step towards highlighting and spoting the defects in system , is the radar that provides cause and effect but its an old method , a rather passive technique.

En este punto recomiendo revisar los procedimientos, la variables y métricas de calidad y la aplicación de los mismos por el personal. Las capacitaciones periódicas permiten que el personal cumpla con las especificaciones.

Es oportuno realizar una auditoria de procesos de calidad porque sirven para evaluar la gestión de la calidad y al encontrar puntos de mejora se tiene la oportunidad de mejorarlos, podríamos decir que fomenta la mejora continua.

La capacitación del personal es clave para mejorar los procesos de calidad, elevar el nivel de servicio al cliente e incrementar la rentabilidad.

Developing the staff is a key of success in an organisation. Agile organisation need people with multi skills. 1st step of the process is a mapping of the skills needed for one function. 2nd step is a gap analysis or self assessment 3rd step is the road map to develop the skills to the right level

Focus on preventive maintenance making sure your plans are robust and adhering to your schedule are imperative to keep your equipment running in perfect conditions avoiding safety and quality issues!

By adopting a Data-Drive approach that could lead to an accurate root cause analysis, organization can effectively mitigate these challenges.

El implementar o mejorar un sistema de gestión de calidad ya sea por Six Sigma o TQM dará a ligar al aseguramiento de calidad que va más allá de un conjunto de acciones si no que es una filosofía de trabajo que reúne acciones y forma de pensar para cumplir el objetivo de cero defectos y desperdicios.
