Indeed having a survey form is just a start, unfortunately most of the survey forms are not used properly. I trust below can be done to improve. 1) it should go to a random sample . 2) each form should be checked in detail. 3) each and every concern need to be addressed with "5 whys" and should get to the root cause 4) ensure both corrective action and correction are taken. 5) inform the feedback giver about the action taction and get his feedback. 6) review periodically. On top of all, building a service culture to ensure everyone collectively work towards one goal will help to practice above.

Yes not all customer are giving same business. Make a group like Platinum, Platinum customers are those who give good business and pay on time. Another one is Gold, Gold customer will give business but not much as Platinum but will pay on time. Another group is Silver. Silver customer will give business but not much as Gold. After making different group we can plan and take necessary action to keep good relation with customer and whom to give priority.

Ask!! Talk to your customers in first person. Irrespective of where technology goes in terms of CRM etc, there will never be anything as vaulable as 'talking to the actual customer'.

Don't create surveys if you don't intend to analyze the data afterwards. Also, no survey will benefit you if you're dead on social media. From my experience, if you genuinely seek quality, honest feedback about your brand, conduct in-depth interviews with your customers. While it may require 3x the effort compared to sending out surveys, the results will surpass expectations.

Don't make the process of giving feedback cumbersome for the customer. Most of the customers don't fill feedback unless its negative. Expected or better than expected service doesn't generally get a feedback.
