Where i currently work we have treatment team meeting daily, and all members of the team are able to discuss concerns, areas of note and then the team works together to address those issues. The providers, the nursing staff, the activities department, we all share our insight and engage in the care management planning for the patients. In addition, most of the team members all have offices on the same floor, so even outside of the structure of team meeting, there is collaboration and information flow.

Likely, the biggest reason providers don't participate in care management programs are because we don't get paid to do that coordination of care. It takes time that is not reimbursable. Providers across the board are overworked seeing patients and there literally just isn't extra time for care coordination, and providers are not likely to engage in care coordination outside of their regular working hours, and that should not be expected. Without time, extra pay, and support, providers will continue to be less involved in care management programs.
