Physicians work collaboratively across a large close-knit network of primary care and specialty providers and regional services with fully integrated technology to streamline the entire process as much as possible.

Promising healthcare delivery models include telemedicine for remote care, Accountable Care Organizations (ACOs) for integrated quality care, and Patient-Centered Medical Homes (PCMH) for comprehensive primary care. Retail clinics, urgent care centers, and digital health apps offer convenience and cost-effectiveness. Value-based care prioritizes outcomes and patient-centric practices. Concierge medicine provides personalized services, while home-based care and community health centers enhance accessibility and address specific needs. AI and data-driven healthcare improve diagnosis and treatment, collectively advancing a more accessible, affordable, and efficient healthcare system.

Self-care interventions for SRHR including family planning in some African countries are providing highlights to support progress toward universal health coverage & meeting SDG 3.7 and SDG 5.6.

In my experience in providing care support service to vulnerable people. The private sector seems to be oblivious of the benefits of integrating care support services for vulnerable people ( e.g the Elderly) into their client packages for effective quality care experience and heightened collaboration that can be cost effective.

Recently I’ve witnessed a patient desperate to stay home with home health care, but unable to qualify because of assets. Not money, but old cars on his property that didn’t run. His insurance will cover a care center, far exceeding the costs of staying at home, not to mention depression from not being in a place you recognize and love. Too much of our system doesn’t consider humanity and quality of life. We need to consider how much more effective in cost and health outcomes keeping someone at home could be.

Patient centricity is a sureshot way to succeed in healthcare. I agree to the model where preventive and pre-emptive medicine is given cognizance. Using technology and data analytics will ensure standardization, improved efficiency along with enhanced quality of care continuum. Though initial costs in such set ups might seem high in terms of tech and training inputs, in the long run they are the real saviors.

This model seems to grow more and more, with telehealth and digital health as tools to enable this model. In the long run, it can be integrated with value-based healthcare and the community-based model, utilizing innovative financing models and resources in the area for more personalized care.

We can use digital avenues to provide high quality care across large patient populations, however ultimately all the right systems should be in place behind the scenes to ensure patients receive the care they deserve from the simple to the complex, and everything in between.
