What are the most important steps to take after completing a safety management system audit?

In my experience, reviews tend to be a lot more impactful than audits. By review, I mean taking a more flexible approach, and looking at effectiveness of the system not just compliance. This has to embrace aspects of culture, capabilities and leadership. It also needs to look at how aligned are views on priorities, challenges and 'what good looks like' across the management layers and departments. Then, actions can be more meaningful and value adding, which means they have more chance of being truly implemented.

Reviewing and reporting after a safety management system audit is crucial for driving continuous SMS improvement. I once encountered an audit that highlighted our safety system's strengths and weaknesses. We shared the report with senior management and the safety committee, emphasizing transparency. The lesson: Communication is key. The report should be shared with all stakeholders, from workers to partners, and it should be clear, concise, and supported by evidence and real examples. This approach not only helped us address weaknesses but also promoted a culture of safety improvement, ultimately strengthening our safety management system.

One thing I found helpful is to be unbiased. Recognize and celebrate the successes and positive results. Communicate areas of opportunity and the need to develop a road map of success. Because some areas may take some time, resources and planning then your road map would require milestones. These milestones would need to see if you are still on track, feasible and finally celebrated.

Set a top 5 priority. Review the audit judicially. Do not leave the meeting until the 5 top safety items are designated and assigned responsible personnel with completion dates for compliance. Set the time and date of the next meeting immediately. Assignment of the next 5 items must be made. Allowing such important action to languish is the #1 reason safety requirements and programs fail.

After collecting SMS audit data, which must be done with complete accuracy and honesty, a review of the data is the first step in creating continuous improvement. Along with other managers, I was tasked with a daily SMS audit that did not have any accountability past completion nor transparent follow up on findings. This neglect harbored a culture of 'completion for compliance' with some managers even completing fraudulently without actually conducting the audit.

Absolutely, implementing the action plan is a critical phase of the audit process. It's where the organization puts its commitment to safety into action. This involves not only executing the planned actions but also providing the necessary support, guidance, and resources to those responsible for carrying them out. Perhaps "implement the action plan" is one of the most unoriginal things to do after an audit, but it is amazing how little effort some organizations put into this. The amazing audit, the well-crafted report, the beautifully written recommendations and the punchy action plan all count for nothing without implementation. As a consultant, one of my measures about whether a client will implement recommendations and actions, is to listen for the phrase "we will implement all the recommendations". In my experience this means little will happen! A client pushing back on some recommendations, having sharp intakes of breath on others, are to me signs that they will implement most recommendations.

The implementation must also be relevant. If the issue identified is forklift safety belt usage, only the affected parties should be introduced to the controls. Otherwise, you risk diluting the message of of the control.

Self improvement is an ongoing journey. A specific instance comes to mind when I recognized a weakness in time management. I often found myself struggling to prioritize tasks effectively. To address this, I decided to implement a time management system, which involved setting daily priorities and blocking out dedicated work periods. It was challenging at first, but with consistency and regular self-assessment, I gradually improved. The important lesson here is that recognizing personal weaknesses is the first step. I think not all audits require a formal follow-up audit by the auditing organization to verify nonconformities or opportunities for improvement. For example, there are audits that do not warrant the auditee to act on the audit results, cases where auditee actions do not need to be verified by a formal follow-up audit, and situations where the audit program manager believes the audit process concludes with the audit report.

Auditor selection Individuals with relevant experience and knowledge in the areas being audited should be chooses for successful audit. Internal auditors can be from within the organization or external consultants. Auditors should have necessary trainings, especially if there are specific standards or methodologies to follow. This ensures consistency and competence. Expertise and competencies required for each audit area should be considered. Ensure that auditors have the necessary skills and knowledge. Auditors should have independence and objectivity. This might involve rotating auditors or ensuring they are not auditing areas where they have direct responsibility.
