This technique involves asking the 'why' question repeatedly (usually five times) to get to the root cause of the problem. By repeatedly asking the 'why' question, deeper layers of causes can be uncovered until the underlying issue is reached. When using this technique, it is important to involve relevant stakeholders, collect data and systematically analyse the information. In addition, considering the wider context and potential interactions between different elements is key to uncovering the true root cause.
