Index is key. Even if the query runs in little time or seems fast, if it is not using the correct optimized fields it could be more efficient. Specially on spatial dabases like postgis, where you want to cross polygons and create complex geometries. Check if the columns of your searches have the correct index. Normally a btree on the fields you query most and certainly a gist on the geometry. Next take the most out of your index, for geometry, look at postgis cheat sheet. Using a bounding box operator like >>, && or ~ as the first where condition will make a difference. The st_intersects use the index, st_intersection does not, put both in sequence and it will be faster. Understand your query and how the database delivers your result!

The most glaring thing that points to a performance problem is increased query execution time, but it's not the only one. You also have resource usage and page reads. In SQL Server, you have various options, ranging from running the query in SSMS (if you know which query is performing poorly) to either finding it in your monitoring tools, in the plan cache using sp_BlitzCache with @SortOrder = 'avg duration', in the query store directly or via sp_BlitzQueryStore, or from both places using PSBlitz. All 3 tools also provide the execution plans as well as info about potential issues like missing indexes, nonSARGable, simple plan going parallel, etc. Using the same tools you can also find queries based on resource usage and 8KB page reads.
Another way to identify slow SQL queries is to analyze the query execution plans. These are the steps that the database optimizer takes to execute a query, such as scanning, sorting, filtering, joining, aggregating, and indexing. You can use the same tools as mentioned above to view the query execution plans, or use graphical tools such as SQL Server Management Studio or Oracle SQL Developer. By analyzing the query execution plans, you can identify the most expensive operations, the indexes used or missing, the join methods and types, the cardinality estimates, and the potential bottlenecks.
Once you have the query and its execution plan, it's just a matter of time until you identify the root cause(s) of the increased execution times. Look for operators that account for a large percentage of the overall plan cost. Compare estimate vs actual records and see if stats need to be updated. See if index scans can be turned into index seeks (note: not all scans are bad and not all seeks are good). Identify potential key lookups that can be eliminated. See if the query might need to be re-written to make its WHERE clause SARGable. Then, in a dev/qa environment with similar data make the necessary changes, flush the buffer and plan cache and re-run the query to see if there are any improvements. By using these indicators, you can compare the performance of different queries, identify the queries that consume the most resources, and detect the queries that cause contention or blocking.
When doing performance tuning in SQL Server, I always run: SET STATISTICS IO, TIME ON; Before starting to test a query and tune it. That way, I can see information about plan compile times, and page reads. I also check the execution plan's XML for extra index recommendations (SSMS only shows the first one if there are multiple) and more info about warnings and statistics that have been used for the estimates. Going back to index recommendations from the plan - keep in mind that the key columns are in ascending column_id order, and not in the order that would be the best match for your where clause. If you're facing deadlocks, but can't reproduce them in your tests, you can get all deadlock related info with sp_BlitzLock or PSBlitz.
You do have to be careful here. For example, if you have user defined scalar functions, using SET STATISTICS IO, TIME ON; will make them seem sometimes hundreds of times worse than they are. Query optimization techniques include indexing, query rewriting, table partitioning, statistics update, bind variables, cursor avoidance, normalization/denormalization, query caching, result set limiting, avoiding wildcard searches, and database configuration tuning.
Before starting any optimisation efforts, set a baseline and then assess both the impact and the gain of each optimisation adjustment made. This is a "game" of juggling, a database with all possible queries fully optimally indexed will probably be hard to maintain.
As a bit of a sidebar, your "longest running" queries are usually not your biggest performance or resource issues. I wrote a little proc more than a decade ago to "show" those worst queries. It looks at the sys.dm_exec_query_stats dmv to figure that out with the understanding that dmv is cumulative since the last restart or the last time that the query was blown out of memory and reloaded. It's similar to the performance reporting available in SSMS when you right click on the instance name in the Object Explorer of SSMS and select reports (and follow your nose after that). If you have ten's (or more) of servers, you should buy some good monitoring software to help out there.
