1 Understand the sources of dissatisfaction

In order to address physician dissatisfaction with compensation models, it is essential to understand the root causes of their dissatisfaction. These may include lack of transparency and clarity about how compensation is calculated and distributed, inequity or unfairness among different specialties, groups, or individuals, inadequate or inappropriate incentives or penalties for quality, value, or efficiency, mismatch between compensation and workload, expectations, or goals, and insufficient or unstable income or benefits. To gain further insight into the issues at hand, surveys, interviews, focus groups, or other methods can be employed to gather feedback from physicians and identify their pain points, needs, and preferences.

I could not agree more. The first step in designing an effective physician compensation model is to begin with questions and not answers. It is critical to not only interview the physicians, but to also include other stakeholders in the organization - medical group administration, physician compensation professionals, and finance. Getting each group’s input allows for a more well-rounded understanding of the sources of dissatisfaction. Seeing the full picture will allow you to consider potential system-specific, design-specific, and provider-specific barriers that need to be addressed before effectively designing a functional compensation model.

Evaluating your current compensation model is important as you consider the design of a new model. The current model is often not without its own merits. Elements that are effective should be retained as it will allow for great acceptance of the new model. In addition, organizations should calculate each physician’s compensation under the new model prior to implementing it. This will enable the physicians to understand the potential impact from the new model on their compensation prior to initiating the new design. The unknown can be scary. Keeping as much of the familiar as possible while demonstrating the impact from the new model will facilitate the transition to the new compensation model.

3 Design a new compensation model

The third step is to design a new compensation model that addresses the sources of dissatisfaction and aligns with your organization's strategy, culture, and values. To involve your physicians in the design process and gain their buy-in and support, you can use a collaborative and inclusive approach. Consider utilizing a compensation committee, steering group, or task force to represent the interests and perspectives of your physicians. Expert advice and guidance can also be provided by a compensation consultant, lawyer, or accountant. When designing a new compensation model, some best practices include defining clear and measurable goals and objectives, establishing fair and consistent criteria and formulas for calculating and distributing compensation, balancing salary, productivity, quality, and value trade-offs, aligning incentives and penalties with desired behaviors and outcomes, communicating the rationale of the model to physicians and stakeholders, monitoring its impact/effectiveness, and making adjustments as needed.

The final step is to implement and manage the new compensation model and ensure its smooth and successful transition. A phased and gradual approach can be used to introduce the new model, while a transparent and responsive system can be used to communicate and educate physicians and staff. Additionally, adequate training and support should be provided for understanding and using the new model. Clear and timely feedback and reporting mechanisms should be established to track performance and results, while recognizing and rewarding physicians and staff for their achievements. Furthermore, solicit feedback from physicians and staff to improve the model, then celebrate successes with stakeholders.

I would also add that often organizations will provide guardrails to smooth a transition to a new program to give physicians and APPs time to adjust behaviors to ensure success (both their own and organizational success) under the new comp program.
