I have seen a situation where over 250 untrained staff obtained Enterprise admin rights through group nesting. Though membership of over 50 nested groups and poor group management. RBAC can help here. Though thought about the RBAC structure, etc., is required. To simplify maintenance of the rights being granted.

The most common challenge is that not enough thoughts are put into IAM. Lot of times IAM is termed as AD and AD as IAM. The second most common challenge is access creep where no proper auditing mechanisms are in place and IAM cleanup is a big issue. The third challenge is mitigating the risk of service accounts which accumulate over time and pose a big risk to the whole IAM solution. The last and most important is PAM if not handled properly can pose the biggest challenge in managing IAM.

Your IAM processes, however, need to be integrated and operate effectively in conjunction with other (layers) of security that you apply to your infrastructure. Additionally, you will need to review and revise your IAM and related polices and procedures on a 'regular' basis. To ensure you are protected/ able to respond to ever evolving security threats.
