1 Ethical and practical dilemmas

One of the main challenges of conducting research on CISM is balancing the ethical and practical aspects of the study. On one hand, researchers need to obtain informed consent, respect confidentiality, and protect the welfare of the participants. On the other hand, researchers need to access the target population, collect data in a timely manner, and avoid interfering with the ongoing crisis response. These dilemmas may compromise the quality and quantity of the data, as well as the feasibility and acceptability of the study.

Whilst I believe that it is highly advantageous to have systems and processes in place there is always a danger that one size fits all as people are all individuals. One definite barrier can be especially when the research is commissioned is that Employers will receive feedback on the employees state which they fear(sometimes with reason) that they can lose their livelihood. Clearly there needs to be a great deal of empathy sensitivity and emotional intelligence when gathering data.

Another challenge of conducting research on CISM is dealing with the heterogeneity and variability of the phenomena involved. Critical incidents are diverse in nature, scope, intensity, and duration, and they affect different people in different ways. CISM interventions are also diverse in their types, components, formats, and delivery methods, and they may have different effects on different outcomes. Therefore, it is difficult to compare and generalize the results of CISM research across different contexts, populations, and interventions.

I’d argue that CISM interventions (as originally designed by Everly & Mitchell) aren’t diverse in type at all. There’s actually a high level of fidelity within the original model. Rather, the various dilutions and disseminations of their original work by other people taking their ideas and calling it someone else (for example, the original critical incident stress debriefing has been used under the guise of critical incident debrief, or critical debrief etc. A simple rule of thumb is that if it’s not Everly & Mitchell’s model, as taught by ICISF, then it’s categorically not CISM, it’s something else.

3 Methodological and statistical issues

A third challenge of conducting research on CISM is addressing the methodological and statistical issues that arise from the complexity and uncertainty of the field. For example, researchers need to choose appropriate designs, measures, and analyses that can capture the dynamic and multidimensional aspects of CISM. They also need to control for confounding factors, such as pre-existing conditions, coping strategies, social support, and exposure to trauma. Moreover, they need to deal with the problems of low response rates, high attrition rates, and small sample sizes that may affect the power and validity of the study.

There is a real danger of jumping to conclusions on limited data as above and other external factors that may well prejudice the conclusions drawn( money, prestige, fear, thanks & recognition) Heightened awareness of the weaknesses of the research balanced against doing the work and seeing what nuggets of useful information can be uncovered and applied to improve matters.

It’s literally impossible to establish adequate and sufficient controls for the whole range of confounds that may occur within the objective nature of, and subjective experience of a critical incident. They’re all inherently different across an expansive range of diversity, as are the people experiencing them. It is however really simple to ask people about simple data sets, such as - did you find this helpful? Too much academic psychobabble doesn’t help first responders.

4 Theoretical and conceptual gaps

A fourth challenge of conducting research on CISM is filling the theoretical and conceptual gaps that exist in the literature. Despite the popularity and widespread use of CISM, there is still a lack of consensus and clarity on the definitions, models, mechanisms, and outcomes of CISM.
