During my 42+ Years in Petroleum Upstream Operations I unfortunately witnessed a number of severe safety incidents where serious injuries occurred and in several incidents there were fatalities. In every one of those incidents there were short cuts taken or inattention to surrounding conditions. Ideally: implement a safety culture, both physically and mentally, where everyone is respected and treated as a valuable employee that is doing their best, where it is ok to speak up and share mistakes so that everyone can learn and improve. Become a learning organisation where mistakes and failures are seen as opportunities to improve and advance, rather than an opportunity to punish and belittle. Fail forward!

A commonly overlooked impediment to the formal reporting of incidents and mishaps, is the amount of time and effort that the reporter will have to spend in the process. Not to mention the general hassle that results from a formal incident report. I know for fact that this once prevented the course director to follow up on a report about a mishap ( a rubber raft that got stranded and completely filled with water resulting in ao cameras, passports, etc., getting soaked) that I wrote in my role as a instructor/contractor in charge of that trip. Top Management support and commitment to safety: If there is no commitment from top management to reporting, participation will be poor. 3. No reward and recognition system to encourage people to show commitment. 4. Non availability of standard and simple reporting tools.

In my experience When the team has a correct understanding of the importance of reporting safety cases, they are encouraged to do so, they should know that these reports primarily help their own health and, in the next step, make the work environment dynamic and safe, and if their reports and If the effects of these reports are communicated to the management and they are appreciated, we will give more motivation to the employees

Continuous training for employees on the significance and proper methods of incident reporting is essential. Equally important, and often overlooked is the emphasis on how these reports lead to enhancements in safety measures.

I've worked with a company, and seen an incredible strategy: They have Two boxes fixed at different locations of the operational site, one box contains the empty safety cards and other with the filled. Anyone of the employees can fill in the card and report an incident or unsafe acts at the operational site. A safety engineer checks the cards box on daily basis and prevents the cause of unsafe acts. Moreover, the person who reports the most unsafe incident is awarded at the end of every month with an certificate of appreciation and a piece of cake. :) Act Safe - Be Safe

Near Miss/hit Reporting can be a valuable tool in fostering a culture of no blame. Employees can be encouraged to engage with reporting with various reward schemes that acknowledge improvements to safety led by Near miss incidents. Employees should also be encouraged to give their input in to how the organisation can reduce risk based on near miss reports.

It Should be a valid reasonable reporting and same can be treatable with a appreciation or rewards program to ensure quality reporting

It is not enough to report, but reported acts, conditions and incidents should be investigated, mitigation and control measures provided and lessons learned shared so as to enhance a positive safety behavior and culture ??
