What does an emergency medical technician do?

In my experience, the training that EMT's go through is invaluable not only in the public realm but also in the private sector. Those who are trained can be of significant value in the contracting realm when assignments take contractors into high-risk environments abroad. Companies that manage employees who conduct business in high-risk environments should always consider assigning those with EMT backgrounds to those projects. Personnel assigned can really make a difference.

Because of the clinical skills, leadership requirements and environments that combat medics receive in the military; many are ready to serve as experienced senior EMTs.

It's important to note that the specific scope of practice for EMTs may vary depending on the level of certification (EMT-Basic, EMT-Intermediate, EMT-Paramedic) and the jurisdiction in which they practice. I have been involved in EMS for 33 years. Having served as a Paramedic since 1998, it is one of the most rewarding jobs I have ever held.

Nationally, EMT-PARAMEDIC is a term no longer used. Instead, simply "paramedic" is the professional term. This change occurred in the early 2020s.

I started my public safety journey as a volunteer firefighter and became an EMT-B shortly after that. At the time, the only certifications were for EMT-B and EMT-I the precursor to becoming a full-fledged Paramedic which I briefly considered. EMS has so many different aspects to it. Responding to emergencies is only one aspect. There are plenty of private services that do a lot of transports, from a basic level to a MICU level. There is always room to be better and always something else to learn.

It is critical that EMTs possess a variety of skills and qualities to perform the job effectively. The combination of physical, mental, emotional and ethical skills is crucial to dealing with the complexities and challenges inherent in emergency care. Your ability to think quickly, remain responsive under pressure, and maintain a high standard of ethics and professionalism is essential to providing specific care and satisfaction to people facing emergency situations. EMT should be physically fit and mentally stable. EMTs encounter different scenarios, some will be challenging scenarios difficult to handle at times. They sometimes suffer from traumatic stress disorders. Personally I feel that they must do a counseling course after EMT certification completion.

The basis of emergency care, in any situation is the ABC’s. Airway, Breathing, Circulation. Any level of an EMT, will focus their care on this. Many times a higher trained EMT will forget the basics and try to move on to advanced care measures, but everything will bring you back to the basics. Many situations will see a Basic EMT begin care and EMT-P’s will take over but ALWAYS shall monitor and maintain the ABC’s.

Attention to detail. EMTs need to be quick on their feet and cool headed. Assess the situation ASAP! Communicate thoroughly and make sure you are understood loud and clear. Know how to look, listen, and feel. Whether it be your team, patients, and/or paramedics. Know clearly and confidently what you have to do in order to administer the proper treatment to patients. Keep the patient calm and alive until the paramedics show up. Adapt to any situation. Demonstrate your evaluation and administered care clearly and in a professional manner to the paramedics. Stay fit, it helps combat stress and fatigue. Study often and keep up to date with practices and laws pertaining to EMTs.

It should be noted that most start out with little driving experience and are ask to drive large vehicles with oversized boxes. As a safety person with an EMS agency vehicle contacts are a huge problem. More training needs to be provided before placing new EMTs behind the wheel

At the culmination of the class, students must pass the state psychomotor and national registry examinations. Once these steps are completed, students are officially EMT-Bs!

Education and training for EMTs can vary based upon state and local laws. EMT training can generally take place at any location and by any provider that is approved by a state EMS office or regional EMS office. EMT training is generally one semester long if taken at a college, this can be compressed if the training is provided in a rookie school.

The Career Outlook for EMS Providers is hard to tell currently, due to the aftereffects of a Pandemic and the lack of funding for EMS. The demand for EMT's and Paramedics is very high, but the people entering EMS Programs are down, and need to be bolstered. EMS in my days was a great job to help people and help communities. But the pay was low and most of us needed two or three jobs to afford life. Now days some pay scales are coming up, but the pay is still low overall. Additionally, due to the demands many providers work long hours and this is producing physical and emotional demands. The future needs some work and management of the EMS World needs to get moving to ensure EMS is viable for the future.

I'm simply going to use this space to recognize the efforts of people I know who served as military medics. Regardless of what the states or the FED feel about how this translates to civilian work, it does demand the respect of what training is needed.

Doug, with the numbers down, why are professional organizations pushing for more and more requirements to get the certifications? I am all for implenting evidence based practices, but if we are drowning the pipeline, what is accomplished?

The job demands a lot of dedication, that I consider to be a sacerdoce. In my view EMTs are real heroes, so present, so discrete.

One thing we need to think about in Pre-Hospital Emergency Medicine, as we move forward as medical professionals, is the level of education we are offering incoming EMTs. I feel there is currently an over saturation of what I like to call “pay to pass“ EMT schools. We need high quality dedicated EMS education centers that are teaching our incoming EMTs how to be high quality providers and not just how to pass a test. We are actively dealing with life and death and yet many of our incoming students are completely unprepared for the level at which they actually need to operate. I truly believe that our high turnover over rate directly correlates to poor education standards and quality of the education our students receive.

As others have pointed out, you must detach yourself and not become personally involved. There will be those calls when you will question if you had done something different would the person have lived. You must find an outlet for your feelings, be it talking to a significant other, crew members, clergy, or professionals, the key is someone you feel comfortable talking to. For me, I used humor to cope and NEVER in front of the family and NEVER at the scene. Find what works for you!
