You/your team wanted to reach a goal and didn't reach it, or in some cases, overshot it. So then we say, "it's a failed attempt". Notice I said that the attempt failed, not a person or a team. Your view of failure is key. If you've got failure internally mixed up with yours or other's value and worth, then your communication about failure will have a subtext of anger, self-preservation, and wanting to prove something. However, it's ideal to view failure at face value - at the attempt. When it's objectified like that, it's not personal. Then it's easier to be compassionate in your communication.

Communication resolves half of the issues and assumptions. Once the impact is analysed and understood. The communication should take place with proper dialogue where every involved person gets a chance to share their feelings and facts. Giving the room to people to feel heard and valued. It helps to understand the level at which the failure was taken and what can be the lesson learnt. It helps to foster an environment of trust and openness
