This is a critical scenario which requires immediate & positive action. 1. Immediately take the senior management into confidence informing them of the graveness of the situation. 2. Conducr RA & identify root cause using any methodolgy one is confortable with. Involve the workers & the person who may be effected. 3. What immediate measures can be undertaken to mitigate the risk so that stopping work is avoided. 4. If mitigation is not possible without purchases then stop work and arrange for purchases on war footings. look at short, medium & long term sustainable solutions & justify it. 5. Cost in relation to benfits. It is inportant to note that ceasure of work is the last resort. 6. The complete event needs to be recorded.

The worst type of incident investigations is one that you know the cause before you begin the investigation. In my experience, 5 whys is a good method, but if you only ask why in one area, because of your "knowing the answer" when you start the investigation, the process will fail to get to the real root cause(s). You must discipline yourself to look in a the 6 areas that are known to cause safety incidents (injuries, etc.). Therefore, the 6 area - 5 why method solves the major problem with most investigations.

In my experience as an hourly employee the workers themselves can make the biggest impact on safety operations if they remember one key thing. You’re not doing something safely because there is a rule in place that defines the safe way. You’re doing things safely because at the end of the day you want to go home to your loved ones and you want the people working around you to be able to do the same. First thing is to Contact the safety officer /supervisor to discuss the safety concerns in an effort to resolve them with immediate effect. Supervisors/ safety officers should promptly investigate the concern and take any appropriate corrective action. If the safety officer can't resolve the concern, he/she should contact the employer/manager about the concern.

I found there are so many close calls where and incidents was averted due to timely action but the experience is not shared across the team that keeps the risk alive. We introduced One Page ‘Training Circulars’. The veterans and current staff were encouraged to share their experience and actions that averted an incident. These training circulars were circulated among the team and every concerned member would read it and sign the document.

The full process may end up focusing on the current situation too much and tailor solutions around adjusting what is already in place. It may be more effective to take immediate action that successfully delivers a safe process, and provides more time to follow a more complex and detailed change management approach without the pressure to restart operations looming. An ideal situation could be to bring forward a planned upgrade or renewal ahead of plan in preference to temporary fixes and making do.

Remember that you have the right to refuse work in environments that you feel is too unsafe to be in. PPE is the last line of defense for your safety and the worst type of control to use over engineering the danger out or keeping you away from the dangerous area until safer equipment can be used.
