If you really stop and think about it, in my experience, if you are not doing Safety right, you are probably not doing other things right either. Also, this is probably not the first time the concern has come up, causing employees to focus on this instead of more productive items.

Then I will give the feedback by assessing the urgency of the safety concern , its cause and then make improvements or take action. Finally I will follow up by encouraging the employee to share lessons learnt and improvements made based on the safety concern to fellow employees and also how to use that incident to proactively address potential hazards.

I agree, investigating the concern is crucial and for that, I strongly recommend Root cause analysis 5 whys method. It provides the deep insights of the problem or issue your going to resolve. It also provides the insights of both sides of coin which is very important while investigating any concern. When the root cause is known any problem can be permanently resolved.

One of the lessons learned I've experienced with incident investigation, is to fully explore every avenue even if it seems you have already found the root cause (or so you think you did). Having 100% of the picture can sometimes look different than even having 95% which can change outcomes, it may have been an ineffective SOP that left something out in the event of contingency plans, ensure you fully understand the picture before taking action.

When a safety concern is received, firstly, will appreciate the reporter for the observation then will ask if any intervention was done by him/she. Secondly will analyze the concern which might include physical inspection & verification of reported concern. The mitigation/control will depend on the risk level & affected parties. If the close-out lies with another department, then engagement with the departmental head suffice. Further investigation utilizing 5-why, FMEA, cause & effect analysis might be required depending on the risk. Recommendations, action list, parties & deadline for close-out might be required. Update the reporter on actions taken. Finally, it can be cascaded as safety moment or LFI in meetings for learning purpose

Last week, coinciding with the annual thorough examination of our lifting equipment, a vigilant employee brought to my attention an outdated color tag that had become obsolete, potentially causing confusion among staff regarding the equipment's inspection status as per LOLER regulations. I immediately escalated the matter from important to urgent, conducting an inspection of all areas displaying lifting equipment information and removing any outdated color tags - after which I personally approached the employee who raised the concern, informing them of the actions taken.
