When building a relationship with vulnerable people it is best to first have a real conversation with them, ideally face to face. They need to understand you have their best interests in mind and you must establish a level of trust. Once you have this you can ask them to complete the required (yet very impersonal) documentation. They should now feel comfortable that their information will be handled with the respect it deserves.

Oferecer canais de atendimento que n?o resolvem o problema do cliente, é inadmissível. O atendimento pós-venda é de suma importancia e deve buscar solucionar o problema como um todo, sem que o cliente tenha que percorrer vários caminhos, fazendo vários contatos e com mais de uma area, a ponto de se arrepender do negócio que realizou.

I would encourage people to consider automating as much of these pieces to help ensure that these tips are not missed. A frequent barrier I encounter is costs related to medication. Creating a reminder within the EMR for care managers when a generic medication is available when a name brand medication is prescribed helps remind us to have this discussion with the treatment team.
