It does not alway mean having the camera be the eyes of the audience. It can also be the angle and even have the person who might be in danger smaller in the frame while the threat is large & looming. Think of the fight scene between Vader and Luke in Episode 5. Or perhaps the Dr Lecter and Clarice Starling converstations in Silence of the Lambs. Those are are excellent examples of tension with fantastic blocking, choreography, and performance. The craft is impeccable on all fronts, and it would be wise to study them to learn and improve our stories.

The b-roll id call it an "ab-roll" meaning its not totally a b-roll, thus id show scenes of the characters immersed in something contrary, to carry the story further, so when I get a subjective shot it comes as a surprise, which intensifies the suspense. That way when the audience sees scenes they aren't gonna go "Oh, that guys the killer"

Colour theory is an excellent thing to use to help tell your story but also to create tension throughout your film. Certain colours correlate to certain themes and thus just by using a certain colour such as Red you are able to enhance that sense of danger. A room flooded with red light just automatically looks dangerous/demonic before anything has even happened and so therefore you can very much take control of your audiences mind if you are smart in the way you use colours throughout your film.

Cinematography is so much more than camera angles. I've seen tension established by placing the camera under a table to show the shaking of an actor's knees. A wide scene can become menacing with just a form passing in the background. Panning the camera sharply or slowly - both can increase tension. Cinematographers are limited only by their imagination - if you are a cinematographer living your everyday life, you are working if you are looking.

So many ways, depends on the scene. I think a dolly zoom can be highly off putting, but not necessary cause “tension”. Also, jarring Dutch angles can work great. Ex: Mission Impossible aquarium bar scene.

I know that tension can be achieven by angles, music, expression of close-ups, etc. But what excites me most not only considering the energy while shooting it but also in watching movies - the ability to let the scene be shot in one frame. It has reality of place and time but also of emotion of the actor(s) as well as synhronicity with the crew. It is unprecedented, especially when there is no artificial management of rhythm, tension and pauses, but when all of this is witnessed in a scene recorded in one frame, we have the impression that reality is happening in front of us and witnessing it increases the tension.
