NO!!! Renaming your files based on revision dates/times is a HORRIBLE “best practice” and should NEVER BE CONSIDERED. Changing file names can have a detrimental effect with regard to referencing. It can also cause confusion as an employee may ultimately end up NOT TRUSTING that the most current file name is correct. Deadlines, emails, phone calls, and other distractions can get in the way of remembering to rename a file. This is an absolutely horrible suggestion!

We have a master revision sheet in each project drawing set. This creates an easily managed system of tracking changes to all drawings within a project in one location. Noting revisions on individual sheets get disorganized easily. Only the revision letter of the change is shown on the individual sheet. The format for documenting changes (revisions) must also be defined in a corporate standard for uniformity and accuracy.
