The devil is in the details! I find considerable effort goes into plans for consistent reporting, milestone dates, metrics, and so on. With the proliferation of communication tools (email, text, IM), the little questions (and answers) are often floating about in a text thread between only a handful of stakeholders or an email chain a mile long. Establish one, efficient communication method in the beginning, include the entire team, and adhere to the plan.
