In my opinion, following steps should be good add-ons : 1. Platform and OS versions for mobile applications 2. Repro rate based on device form factor 3. Build links with prod behaviour 4. API call details with response codes 5. Logs for debugging 6. Timestamp of bug reproduction 7. Screenshot/Video 8. Numerically mapping repro steps, actual & expected results 9. Tags to identify the bug wrt release cycle 10. Labelling with Blocker/Critical priority wrt Launch status

Must have Steps to reproduce it 5. Clearly mentioned Expected vs. Actual results 6. Must have Environment details with version of software, hardware configurations used in execution. 7. Must have Console logs, log files, screenshots 8. Must have Bug severity and priority 9. Advanced info (optional). Tips: 1. One bug = one issue. Don’t include multiple bugs in the same issue! 2.Avoid duplicates. Always try to search your current issue tracker for existing reports.

Severity and Priority. 3. Defect Type. 4. Defect Status: with an initial status of 'Open.' 5. Assignment: Assign defect to specific developer responsible for the module. 6. Date of Identification. 7. Steps to Reproduce: Clearly outline the steps to reproduce the defect, keeping it concise and relevant. 8. Attachment: such as videos, images, and log files if available. 9. Build Version. 10. System Details: Give brief information about the system on which the defect was identified. 11. Test Data: Data or files which has been used to test the scenario/case.

Make sure your naming conventions and technical terms work for everyone throughout the development pipeline, the bug-tracking pipeline, AND the delivery pipeline. If your developers use far different documentation language than your business analysts (who often have little to no tech savvy), communication will falter. Without shared language, you're not getting very far in terms of making sure the bugs stay fixed and don't re-emerge.
