What are the best methods to identify the root cause of a production line defect?

I've frequently employed a straightforward yet highly efficient method to identify the root cause of a problem, known as the 5 Why analysis. However, the fishbone diagram is an excellent tool for more intricate issues. In general, the Six Sigma methodology of the DMAIC is highly effective in resolving production line defects. Within the Six Sigma framework, the analysis step includes the root cause identification. Statistical data analysis methods are deployed to understand potential causes in Six Sigma.

In my working, I have realise that a claer understanding and problem statement is what is to start with. This usally can be achieved by an open discussion bringing out the various aspects of the isse, possible causes and probable soutions. Once can use various tools to evalaute the issue and arrive at the root cause. I have found that the fishbone method has given me rich dividends since this can easily accomodate stray/outlayer causes which may be overlooked often.

What “simple steps” don’t tell you is that gathering data and defining the problem is the easy part. The difficult task — the one that requires all the data and evidence — is convincing the next person. That person often has a conflict to “keep going” versus “stop and fix”. Think about how you can define the problem so that it is better to “stop now” than to “fix it later”. Always define your problem including: what it means to our business and our people to fix it now. Consider your car. Is it better to stop in the city now or to gas up later during unknown conditions. You say, “city has better prices” and “We have less worry with a full tank.”

Identifying the root cause of a defect in a production line is crucial for implementing effective corrective actions. The best methods to identify the root causes are, 1. Reviewing FMEA. 2. Cross checking the 4M matrix. 3. Verification of past history for the similar type of defect. 4. Conducting the Is /Is not method. 5. Stratifying the data.

Before analyzing the process and start collecting data you may need to validate/verify your measurement system. This should always be a checkbox before analyzing any data.

Clearly outline what data is needed, focusing on specific aspects like time, location, product type or machinery involved. Employ data collection tools such as sensors, automated systems, or software that track and record production metrics, quality control results, downtime logs and operator feedback. Review past records and historical data to identify recurring patterns, trends or anomalies related to defects. Observe the production line in real-time, examining equipment, processes and operations firsthand to identify potential issues.

After an issue is known to exist usually when it repeats, look back in history,(shift reports, Maintenance log entries etc.) to try and identify any changes or events that may have led to the occurrence. The constant collection of this type of data cannot be underestimated when trying to find a root cause.

The single defect occurrence (like spec. non-complience or operating window excursion) must be investigated. You cannot afford consequences of it's reoccurrence(s), if you would decide to wait and test the level of tolerance to production or process safety losses.

One key point to solving defects is to always "... go where the body is". Related to my Kaizen training the concept is that no detective ever solved a murder from his desk. Instead you need to go where the defect was identified and investigate on the production floor with those who know, interact and live with the process more than you. The production line technicians and workers may save critical time by giving clues that aren't in diagrams, process manuals or schematics. Process changes, interruptions, expired material, anomalies, etc. are most often times noticed by those closest to the process. Engaging these valuable folks in objective discussions should be one of the first things one does depending on the situation.

Go at the floor where the problem is along with your team. Talk to operators, maintenance crew working there. These will give you important cluses for your investigation. From there move on to all the approaches as mentioned by other members

An important pre-requisite before an effective 5 why analysis is to have a clear differentiation between causation and correlarion among the potential root causes. While two metric may have direct correlation, it is not necessary that one is caused by another..

I prefer to use a FMEA for identifying potential causes with highest RPN ratings and then do a deeper dive using 5 Whys x 3 to arrive at specific, detection and systemic root causes for this high risk input failure mode. Before starting on doing an FMEA, an Input/Output Process analysis can be used to identify critical inputs by a cause and effects C&E matrix. These critical inputs are identified by rating them against customer requirements and then analysed using the FMEA as mentioned earlier.

It should not be assumed that the trouble-shooter knows most about the problem. There is often a person who knows the problem and the solution from experience elsewhere. This could be an equipment supplier or service engineer, or an ex-employee or somebody who has been promoted. Whilst advice must be weighed before being used, it is likely to save wasting time and money. Asking advice shows intelligence, not ignorance!

During brain storming session,do not forget to call persons who are in constant touch with the equipment.Maybe operator or maintenance team,they can give correct symptoms of problem and can help to identify the root cause.Involve optimum number of persons ( 10-12)for brain storming ,if there are many it will become a crowd and there is every chances of neglecting significant points from participants.

Testing and verifying the root cause simply can be done by GEMBA. Just visit the line frequently and observe. Ask the operator the most important thing "show me how you do it". Once he does in his routine manner, you will notice the thing / activity that is not right.

Assess the probability of your causes based on comparison data. Use the data to find the most likely causes. Then think about ways on how you can verify them fast and safely. Think about the risk profile of an action. Always bear in mind, every physical action that you take bears the risk of causing additional problems, worsening a situation or to erase critical data which you might need for your root cause investigation.

Now I will use pareto diagram to prioritize the action to be taken by ABC analysis I.e,first attending the higher % one.First we have to concentrate on the first highest one.

Formulate hypotheses based on potential causes identified during the analysis phase. Design experiments or tests to validate or invalidate each hypothesis. Implement controlled tests by changing one variable at a time while keeping others constant to ascertain the impact on the defect. Develop prototypes or models to simulate conditions, verifying the suspected cause-effect relationships in a controlled environment. Utilize statistical tools to validate findings, employing methods like regression analysis, hypothesis testing, or correlation studies to confirm the significance of the identified root cause. Compare outcomes against benchmarks or historical data to verify the accuracy of the identified root cause in the production line defect.

To address production line defects, implement a strategic plan derived from root cause analysis. Assign responsibilities, set clear timelines, and leverage technology for efficient monitoring. Establish KPIs and maintain a feedback loop for ongoing evaluation. Foster a culture of continuous improvement, document lessons learned, and adapt the solution as needed. Regularly reassess the production line to ensure sustained defect reduction.

These 7 steps seem so obvious and simple. So, why don't we solve every problem with a simple meeting, or a modification to the process or work instructions? A big hurdle is to motivate the production worker. How do you get them to change? How they've always done it is their preferred method. Every situation is different, and I have my experiences/ideas. I'd love to hear new ideas and methods for motivating the production worker to make changes. Rewards are common, but should you need to invest in order to make changes?

Work in the occurrence avoiding the failure better than work only in the detection and by consequence do our final controls more and more complex.

#1 Google doesn't have the answers to everything. #2 Peel yourself away from the PC/phone and interact with production managers and various production employees, IE: ask about "suspicious machine functions" or did we "forget something?". #3 Delegate a production manager or otherworldly employee to catch "it" in "the act". #4 Actually visit the production floor and get your hands dirty. It is manufacturing by the way, production employees are either scared of you or love you, listen to them and they will love you to make their jobs easier. #5 Remember machinery is heartless, good employees enjoy your encouragement and assets of safety mindful. Don't overthink it yourself, gather production information from a wide array of personnel.

There is possibility to return back to Basis of Design after fixing of defending barriers. Go on and escalated the Defect to attention of upper management. You'd need strategic support, resources and funs.

Highlight all steps and subprocesses, allowing for a clear visualization of the workflow. Use established problem-solving tools like Fishbone Diagrams, 5 Whys, or Failure Mode and Effects Analysis (FMEA) to pinpoint potential causes and their interrelationships. Test the identified causes to confirm their relevance and contribution to the defect. Verify their impact through testing or simulations. Develop and apply corrective actions addressing the root cause. Monitor the effectiveness of these solutions over time.

Do not fall in love with our own solutions. Allow someone else who did not come up with the solution to monitor and provide impartial feedback. If the solution is not working well according to planned, be courageous and be open to change. As long as we learn from the experience and do not make the same mistakes, it’s worth the trial.

One activity I have found to be helpful just after defining the problem is to Gemba, "The real place" , the advantage of observing supports the problem solving immensely. Followed by a Fishbone and a basic why why analysis . The last " WHY"should often than not, be the ROOT CAUSE , furthermore, the countermeasure deduced can be tested for effectiveness over a period.

In my experience, quality defects analysis can be classified into two types: (1) Event analysis (2) pattern / period data analysis. Event analysis - production or QC person detects a defect and same needs to be investigated to avoid producing more defectives. One will need to use knowledge / experience to understand mechanism behind defect generation. Why Why analysis is another tool to reach to root cause. Pattern / period data analysis - For chronic defect generation, generally there are multiple case and hence one will need to apply standard method like QC story or DMAIC or six sigma process or 8D method. Watanabe classification may also help one identify system level issues leading to reoccurrence of defect.

This subject is much more complex than what has been discussed here. However, there is one concept that is missed completely. While everyone is focused on production issues, the periods when everything is running perfectly go unnoticed. One of my production managers stopped by to tell me that one of the high speed production lines had only 2 minutes of downtime during a 480 minutes shift. He was very proud and so was I. I asked him does he know why? He had a confused look on his face. I suggested to look and record machines settings, line personnel and their level of training, packaging material specs vs actual, product spec vs actual and all other relevant conditions that made the perfect storm. Sort of AAR for line working right.

When you want to find the root cause of a defect I recommend to follow these steps as it will save you massive amounts of money and time and help you find it fast: 1- Map the production process from start to end including the 3rd party inputs 2- Map the defect(s) that you have based on material, production and process categories 3- Sort out the defects from crucial to lower priorities 4- Link the defects to possible step in production based on the facts and assumptions that you have 5- Now you have a matrix that link the defects to steps and you can start investigating them from high probability to lower one Most if the time when you dealt with high probability one your lower probability will be solved or canceled ???

In my experience an important aspect is Gemba walk and talk to the actual doers . This will mostly guide to the actual root cause and the point where near misses were actually missed. Usually, it’s a combination of multiple factors, some of which are underlying conditions contributing towards a failure.
