Identify why the safety hazard was not reported. Lack of training? Boss telling the person not to report it? Trying to cover something up? Don't assume you know the 'why' until you have looked into the situation.

Identifying safety hazards is one of the good practices. No point identifying but not reporting it. we could also review the process on these lapse as the potential injuries could be fatal. Looking at the positive side, the particular member may and could overlooked on the matter due to his busy schedule and he cannot be defaulted. Consistent coaching and briefing to all team members during toolbox meeting helped members in adapting the mentality and importance of the subject matter.

If you have a safety committee (SC) it may be SOP to report the incident to them. Most companies I have worked at this was the case.

Prior to "personally" fixing the issue, I'd have that person either walk, drive, sit, eat, work or stand in the "unsafe area". Then, when they were injured, I'd visit them in the hospital smile and say, "Maybe next time you'll report the safety hazard." I'm just sayin'...

An example of policy reinforcement that I would like to add is of the workplace violence and harassment policy review after any workplace violence and harassment incidents. This policy review can be a part of de-escalation and staff debriefing. The policy review must be follow by the workplace risk assessment and subsequent mitigation for preventing incidents of similar nature in future.

Foster an environment where employees feel comfortable reporting safety issues. Assure them that their concerns will be taken seriously and that there will be no negative repercussions for reporting hazards.
