A subcontractor disregards safety protocols in an emergency response. How would you ensure a safer outcome?

You just can't wait for any accident to happen! Before finalizing any contractor, make policy as per your HSE goals/targets/commitments. If any untowards happens, after analysing the event take action as per terms and conditions whoever helds responsible. Further, share the outcomes of analysis amongst concerned person to prevent repetition in future.

Risk assessment briefings are crucial for the workforce. Instead of merely filing the document away or saving it on a computer, it’s important to actively communicate the risks and control measures to employees. Additionally, periodically review the effectiveness of these control measures on-site.

It seems that one mistake can lead to other critical mistakes. A building was under construction. Two young men on opposite sides were tying steel rebar 15 feet off the ground. They were secured with safety harnesses. One man missed attaching his harness and fell onto a piece of rebar below him that pierced his body leaving him hanging and in intense pain. The crew knowingly had not capped the exposed pieces of rebar with plastic safety mushrooms to save time. These would have prevented such an injury. The safety officer immediately ordered his crew to cap the mushrooms, this as the man was hanging impelled. The mistakes continued, the company fined by OSHA, life long injuries sustained because of one mistake to save a few minutes. Tragic!

Safety in Emergency Response is just as important as responding to the emergency. Those involved in that response can either make the situation either better or worst. Safety protocols have been developed as a result of past actions that had negative results. If I came upon a safety violation, I would hold actions, call for the section supervisor and review the risk assessment done prior to the safety problem to exist. And if that was deemed to be an acceptable risk, why it wasn't communicated to those above, so additional risk migration efforts involving equipment and personnel were not present as a risk minimization measure. While we need to do what we do, safety can never be throwout.

It would depend on the seriousness of the breach. If very serious they would be removed from the site with the potential of being banned. If a less serious breach they would be reinducted into the site and put on a warning. I’ve had to enforce both on varying sites. Safety first!

S.O.Ps in case of emergencies are part of the organizational policies to help achieve their goals. The laid down procedures are better followed always

I have to take a slightly different perspective here: someone in your organization should have reviewed subcontractors safety protocols as part of the vetting process. Someone in your organization should have already looked at training before hiring them.

Continuous practice and recycling are key to ensuring the continuity of the skills acquired in initial training in safety and emergencies.

Training Emphasis: Conduct targeted training sessions with subcontractors on emergency procedures, highlighting the consequences of non-compliance and the correct response methods in high-pressure scenarios.

Our brains lose 60% of these capacities during emergency interventions. The first thing to do is to review the risks with the subcontractor during an initial discussion. Secondly, perform an action plan (8D) in the company's own format to get to the root cause. Work out solutions with the subcontractor: - training - review emergency protocol: always have a 2nd person observing (as in fire-fighting). ...

Incident Review: After the situation is under control, review the incident thoroughly. Investigate why protocols were ignored, what influenced the decision-making, and identify gaps that led to the disregard.

Sub or not, all contractors on site have to work safely. Having a safety program in place that includes training records for all the workers is a huge part of that. Do they know how to work safely? Training records will show that. Are they competent at the work being performed? Training records will show that. Are they equipped to deal in the case of an emergency? Training records will show that.

- Prior to award of the contract, include all HSE requirements in the RFQ so that the subcontractor is aware of all HSE compliance to be adhered to. - Undertaking / acceptance from the subcontractor to comply / implement with the contract HSE requirements. - Apply consequence management for any non compliance.

Discuss the matter with the subcontractor, and, if necessary, their on-site manager. Ask if they were aware of the safety protocols. If you have oversight of the subcontractor, review the scope and requirements of your service agreement with the contractor's organization. The service agreement likely contains provisions about following safety protocols. Require the sub to complete retraining. Management of subs can be tricky based on service agreements, scope of work, organizational structure and more. Ensure that you follow the proper channels to address the situation and emphasize the safety requirements for everyone involved in the emergency response.

Safety is most important one in work place. Training program conducted periodically ensure workers safety.

In my experience, the most useful is to use contracts to ensure training in safety and emergencies, and the continuous retraining of these skills. Supervising the safety aspects of contracts is as important as the execution of the work.

Though this would fall under areas for improvement, critical analysis would need to be pursued to identify the break in protocol. Couples of questions to be asked could be : 1. Has regular drills related to such scenarios been simulated before? 2. Has identified gaps in the drills consistently flagged for improvement? 3. Is the company's induction system for sub-contractors and employees effective? 4. Does the drill protocol form part of the daily toolbox talks?

Making this subcontractor part of the management system, treating it as its own and involving it in all initiatives to improve the emergency response system!!

You need to have someone who can 'bridge' training so that all safety measures are achieved. It has been my experience that the EH&S manager is the best person to accomplish this.
