In my view the miscommunication is simply leads to misunderstanding. In long run it will bring confusion with in team members and breaks the trust. To tackle this, we can initiate some steps like... 1. Establishing a proper communication channel (Like daily sync up meetings) 2. Setting up platforms to discuss on ambiguities (Open house) 3. Regular connect with team members (One to one discussions)

My experience says when miscommunication leads to quality issues we need to cross check quality culture of problem solving, accountability and responsibility. here are some key steps you need to take into consideration. 1. identify where communication goes wrong by conducting root cause analysis focus on process instead of blame game. don' forget to involve all applicable stakeholders. 2.check and ensure that team is having clear, precise and concise communication.Use standard template to avoid further damage. 3. provide clarity on communication and role and responsibilities check RACI matrix and train and communicate if there is any ambiguity. 4. communicate lesson learnt and check feedback from stakeholders. and finally review continually
