It is really important to use methodologies that promotes psychological safety, for example a simple technique that I learned is the "stop the world" bucket, where any participant that feel uncomfortable can add a red ball during the breaks to inform that someone is not OK, since it is anonymous, and then after the break the first point would be to recap and see what is the reason there is any potential personal issue raising.
