How do you update and maintain HAZOP Study documentation throughout the project lifecycle?

You should define frequency for HAZOP Study of the units as per regulatory guidelines at your location. As per frequency keep updating basic information of Study time, scope, recommendation status and action identified updates in drawings. All HAZOP study reports should be kept at some folder accessible to assigned persons only to maintain the quality. There are so many IT tools available nowadays with organization so you can look into that. Try to get regular updates from that folder about the current status of recommendations and also schedules of upcoming studies. Always keep the status available in case of any requirement by management or government authorities in the form of presentation or excel sheet. Hope this will help.

Keeping the HAZOP / LOPA in an "evergreen" state solves many issues. Evergreen in this context simply means keep the study under revision control. As new information arises, update the study via revision control. In this manner once the capital project work is complete the same master study can be delivered to the site and maintained in a similar fashion. Thus, instead of having an initial study on Day 1 from the project team and an updated Reval Study 5 yrs later, with many many misc update files throughout the years that need to be reconciled / consolidated at yr 5 instead you have an evergreen master study that aligns as-built equipment in the field at all times.

Also, updated HAZOP report should be shared with all the stakeholders, specifically those operating the plant so they are aware of any new findings that could impact the operation and safety of the plant.

Keeping the HAZOP / LOPA evergreen allows one to track leading indicators against HAZOP / LOPA assumptions and get real-time insight into actual risk profiles.

HAZOP effectiveness can be improved by carrying out Baseline PHA and Cyclic PHA to identify new hazards at regular intervals.

If one adpots the "everegreen" approach study will always be update to date as part of the overall revision management process.

One good practice (should be a requirement) that many miss about is to always referring to the current HAZOP document when dealing with technical management of change. Many start HAZOP from blank paper instead of revisit the current HAZOP that may already reviewed the other portions around the change. Always check the current HAZOP then adjust as per change scope. The practice will greatly help future HAZOP revalidation process.

Often multi disciplinary teams needed for conducting these activities are difficult to assemble. There are other activities like alarm management , RAM, documents needed to develop control system configuration ,Development of O&M procedures. The list can go on. These is where I see use of NLP (Natural Language Processing ) can help. May not be perfect but can provide a better second line of support to bring some context between many of these activities.this will also ensure consistency between the documents .

HAZOP studies should be before going ahead with any project. Moreover, HAZOP and HAZID studies should be conducted periodically for any processing plant as modifications are done throughout the life cycle of the plant. HAZOP should be the part of the Management of Change for any modification and its recommendations should be closed before conducting the Process Safety System Review (PSSR) which is a prerequisite for commissioning of any project

One of the challenges for newly operating facilities is to get the editable version of reports and worksheets from the EPC contractor who conducted the initial study during EPC stage.

After the session, the report must be updated to reflect new findings. This includes revising P&IDs, operating procedures, and manuals. Clear documentation ensures all stakeholders are aware of process changes.

All the HAZOP recommendations shall be addressed prior to closing the pre-commissioning phase of the project and incorporate the accepted recommendations in the project scope. Some of the recommendations may lead to preparing or updating SOPs after the commissioning is complete, but these points shall be discussed with the Operations Team prior to handing over the project, so they are addressed and closed as well.

I have seen some projects where HAZOP recommendations have not been closed before commissioning. This is a waste of resource and dangerous to human life and asset.

Hazop close out is one of the most important component of the Hazop exercise. As a result of the hazop workshop there will be multiple recommendations that need to be properly implemented on site or plant and same needs to be verified by an internal or external technical auditor. In order to reap full benefits of the exercise. A closeout report to be developed post implementation of all the hazop recommendations.

Key concerns for HAZOP over the life time of any project is how to maintain it over time. There are many prospects here 1) Aging and how is the maintenance and inspection programs application.(Reactive, Time based or Proactive). 2) Change management in general and management the change of throughput composition. 3) Revision of the ALARP conditions every major hazard scenario accident. 4) Risk management for every defeating of safety function to make sure residual risk is still manageable. 5) Operational safety audit (External fully supported from 1st level management). 6) Build and review operational safety case every five years. 7) Monitoring every breaching of safety boundaries and do assessment or fitness for purpose study if required.

Maintain a Hazop register with summary of all recommendations and actions with dates of incorporation. It should be given a document number and added in the technical document register.
