Defects are to be detected in a shift left approach. The earlier you detect a defect, the cleaner the code you promote. Sdlc lifecycle checks should fail your pipeline unless the defect is triaged. For urgent production fixes, the risks should be accepted by a manager / director who should be held responsible for getting the risk mitigated within a stipulated time.

Based on my experience, conducting a RCA (Root Cause Analysis) is crucial after validating a detected observation is a valid defect. and documenting the RCA findings, will enable us to understand: a) why the defect occurred on the 1st place b) how can we prevent similar occurrences in the future and c) what additional precautions can be taken to avoid such slips. After resolving the defect, it is very important for the team to highlight the impacted objects/areas and allocate time for regression testing with sanity checks to prevent the introduction of any new defects.
