Whenever assessing for safety, it is important to use your skills. Case managers have an innate ability to identify risks and potential crisis. Risk assessments have its place, but working with the Medicaid population there are many times where red flags are raised just based on the type of clients you are working with. I agree utilizing evidence base case management models is ideal including practical. The models have been researched and you can select the model that meet the needs of the population of consumers being served. I do CDI at my hospital and safety and quality is heavily related to identification of social determinants of health (SDH) and its proper documentation in our EHR. Most doctors or case managers do not identify they are working on SDH despite doing it all day, multiple times a day because some do not see it as the new frontier, the new concept that embodies the core of their job as the person that makes sure a safe transition to home happens at discharge.
